Where do you begin?

Patients who have Medicaid or any Medicaid HMO's such as Simply Health, United Healthcare Staywell or
Sunshine, CMS or any Healthy Kids plan will need to obtain a script from your child's primary doctor. You will need a
referral from your primary care doctor with your commercial insurance plan also. The
physician can fax the script/referral to us at 407-265-9590.

Once the script is obtained by our office we can obtain authorization for the
evaluation and then an appointment can be made with the exception of United Healthcare members.Your PCP will need to start an authorization with UHC directly.

At the first appointment please bring your insurance card so we may obtain a copy.

If you have a co-payment or deductible, payment will be required at the time of service.
For your convenience we accept Visa, Master card, Discover and America Express and HSA/Flex cards.
We also accept personal checks and cash.
