| Where do you begin? Patients who have Medicaid or any Medicaid HMO's such as Amerigroup, Staywell or Healthease will need to obtain a script from your child's primary doctor. You may need a referral from your primary care doctor with your commercial insurance plan. The physician can fax the script/referral to us at 407-265-9590 with your child's general information. Once the script is obtained by our office we can obtain authorization for the evaluation and then an appointment can be made. Patients who have a commercial insurance plan such as Aetna, Humana, etc. should obtain a script and referral which can be faxed to our office at 407-265-9590 with your child's general information. Once the script and/or referral is obtained by our office we can schedule an appointment. At the first appointment please bring your insurance card so we may obtain a copy. If you have a co-payment or deductible, payment will be required at the time of service. For your convenience we accept Visa, Master card, Discover and America Express. We also accept personal checks and cash. It will be your responsibility to notify our office immediately of any insurance changes as we must obtain authorization PRIOR to therapy. Keep all explanation of benefits from your insurance carrier. |
| What to expect? The evaluation will take approximately one hour to complete. The therapist will provide you with a case history that will ask you questions regarding your child's development and other important information. The therapist will use standardized and non-standardized tests, observation and parent/physician information to get an overall idea of your child's strengths and weaknesses. Once the evaluation is completed the therapist will discuss preliminary findings and offer some recommendations. Within 5-7 business days you will receive a copy of the evaluation by mail. A copy will be sent to your primary doctor and a request for therapy from your insurance company will be made. Once the authorization is obtained and the doctor has approved the evaluation, we will call you to schedule an appointment for therapy. Every attempt will be made to find a mutually agreeable time for the appointments. Generally therapy will be scheduled on the same days and times each week for consistency. After each session the therapist will review what was covered in therapy that day and activity recommendations will be made for the home. Every 6 months your child will be re-evaluated to determine progress made, decide if additional therapy is needed and modify or add any new goals. |
